Registration Forms

• Credit/No Credit form (submitted via the Graduate College Student Portal)
• Late Course Change/Late Registration form  (submitted via the Graduate College Student Portal)
• Graduate College Registration Withdrawal/Cancellation Form
• 597 Individual Study Form - Individual Study form(s) can be submitted electronically (via My.SiebelSchool) only for the current and next semesters after your time ticket becomes active for the semester you wish to submit the form.  Please review the Guidelines for CS 597 Registration first.
• PhD Program of Study form

Theses & Dissertation Processes

• CS 599 Advisor Agreement Form (Electronic Form - Recommended) Paper Form, for limited use only if you have a co-advisor or are changing advisors.
• PhD Qualifying Exam Statement Form - MS Word / PDF version (must save form first, then you can edit)
• PhD Preliminary and Final Exam Committee Request
• Master's Thesis/Dissertation Approval Form (TDA):  Master's TDA

Transfer Credit Requests (from Illinois or other institutions)

1. Please get in touch with the CS Graduate Academic Office for an initial review of your request. 
2. Do NOT submit a Graduate College petition until the Siebel School of Computing and Data Science Graduate Academic Office reviews and approves your request.

Change of Course Level Form

• Change of Course Level Form (primarily for students in the BS-MS and BS-MCS programs)
